Just about any website / sponsor you promote will have the 2257 links at the bottom of their pages ( on the paysites ). You don't need the actual model id / release form if using sponsor content. If you're buying content though, it will be delivered with the content itself.. then you just need to keep it on file and provide an address or some other form of contact should the men in blue decide they want to hassle you. I think legally youre supposed to provide a physical address and be at said address during 'normal business hours' but most people just provide a means to be contacted and I've never heard of anyone having any troubles.

Yep! That's the law. You don't get listed in many places if that info is missing plus it's for your own protection to keep records.

I usually just link to it, except when I'm building free sites, then I make a copy.

The 2257 link only needs to be on your index page.
