You can still get a hard on while fearing for your life. It is a sexual organ and can still use the adrenline of fear to give the appearance of sexual arousal when simulated.
That is why it is hard for guys to come forward when raped, they feel guilty thinking that they actually wanted it since their body betrayed them.
