spam blocker?

Ok well I'm using outlook express some my say why and for me it's simple it does the job and I hate installing more crap then I have to. Anyway the spam blocking features are shit. Anyone know of a free software with a built in blacklist? I would like one that is rather smart and I dont have to tell it much, but the black list gets updated and downloaded on a reg basis. I downloaded a couple before I rebuild my system and they are not much better. I still have to tell the darn things who is an who is not spam. I have seen some of the WM use a service where you have to go to a site and confirm you sent a mail which seems nice being I dont have to manage it. Any ideas? New system would like to stay as clean as possible and this business makes that tuff with mail.
