Just a thought, you may want to bump that URL length up a bit
<INPUT TYPE="TEXT" NAME="URL" VALUE="http://" SIZE="50">
Maybe to 70 or 75
Also the text is set to 50, good for TGPs but I think most people try to use 10 words for link list descriptions.
