Luckily my accountant is right across the hall, so I got him to fill it out and I faxed it the day I saw the warning....
Here is the whole warning if any are interested

W9 Form Required on Account.

The IRS has required us to get a completed W9 form on file for all US accounts. If the form is not received by 12/31/03, we will be required to withhold 28% of all payout and submit this payment to them.
