http://www.ev1servers.net/ and I'm pretty happy with it, but I have to manage it myself. These guys from
http://www.servermatrix.com/ also a great price and they have management plains you can choose.

Edit: though EV1Servers doesn't manage your server I have nothing to complain about their support.
