I use http://www.globalcharge.com they take Switch etc. as well as Visa/MC.

They pay 35 days after the end of a month directly in to your UK bank acct. Here's their phone # 020 7517 8800 I speak to Simon Coates.. They work like CCBill and set up all the .htaccess for you as well, plus I use their SMS for a £2 x 30 minute trial which converts now and again and their dialer
