Changing hosts should not effect your page rank nor make you have to resubmit.
Your host being down for a prolonged period could cause you to get dropped.
You might try whois'ing them and getting their number or you could try the wayback machine at
http://www.archive.org/web/web.php to see if they have an archive of their site and get a number that way. I did try this and pages come up for them. Try the most recent pages first and work your way backwards to see if there is a number for them.
