How to make affiliate id manually enter into code

O.k, I am attempting to make a hosted gallery, only I don't want to have cookies automatically log their code in (too complicated for me but if one of you could explain that also it would be great)

So I want the users to enter it manually,

O.k, I need to get it to where when the person manually puts their ccbill I.D in the X's that it inserts that ID into the code of the website.

Do you see what I'm saying?

A good example would be this site here....

You see when you put in your number it autmatically puts that number in the link, yet when I do it, nothing happens, am I missing something here????

Please help.
