Is the main purpose of clean free sites to fill up empty spaces? I guess I never understood them.

Mainly it's a way for a new LL to quickly fill up the categories so your not inviting surfers to bare pages. It's a win-win; convenient for the LL owner just launching and some free exposure for the freesite builder.

This is going to sound like a really stupid question, what's a clean site? Is it non-adult, or something else?

"clean" means a copy of the freesite with no recip links on the first page. A lot of freesite builders make one for their own hubs or lists. It doesn't get submitted to link lists.

oh wow, thats a lot of effort, I'd rather just stay away from reciprocal links altogether. I prefer links in content or site reviews.

Sorry, I just had this image of new LL's filled up with non-adult sites, *shudder* Like ponygirl said, it's one with no recips, so you get a "clean" version without the recip table and you're sending surfers into the freesite without "leaks".
