Need Flash Player HTML for wmv streaming

I have seen so many sites using flash player to stream their windows media video and yet I cannot find the applet or html other than this one below, but it's very wierd on how it calls up the video. there is now .wmv extension and I cant figure it out.

Does anyone have the html that I can just add the video url and stream it from from an embedded player?

Here is the one I have...maybe it's an easy fix if anyone knows?
