I need a php script built

To start it would be a simple form to record data into mysql. The form would be spread over 2 seperate pages. As in enter some data into a few fields then hit the next button to go to 2nd page where the user enters more data and hits submit.

After submitting their info they would recieve a unique referal url. kind of like an affiliate link. This is used to track who sends referals. perhaps it would send an auto email to the referer once the user who used that referal link meets certain conditions.

What is involved in building a script like this, is php adequate? how long to build and roughly how much would it cost?

I'm sure someone already has the bits and pieces, just need to combine them to do what i want.
