I need a copy of a model release

Here is one we have used, it's pretty basic



MODEL RELEASE

In return for the sum of $............ which I have received I hereby give *** hereafter known as the photographer, the absolute and irrevocable right and permission, with respect to the photographs, videos and/or digital images he has taken of me and/or in which I may be included with others:

(a) To copyright the same in his own name or any other name *** may choose.

(b) To use, re use, publish and republish, convert to digital form, print from digital form, digitally transmit, store and display the same in whole or in part, individually or in conjunction with other images, in any medium including (but not by way of limitation) illustration, promotion and advertising.

I hereby release and discharge the photographer and *** from any and all claims and demands arising out of or in conjunction with the use of the photographs videos and/or digital images including any and all claims for libel.

This authorisation and release shall also endure to the benefit of the legal representatives, licensees and assigns of the photographer.

I also confirm and agree that the photographer has at all times acted in a correct and proper manner.
