Excerpt with Thumb

On one of my blogs i am trying to set it up so it shows excerpts on the main page but with a little thumb for each post, thumb of the picture used in the post. I know WP makes the thumb, it's in my upload directory, but getting it to work with my theme, I can't seem to get it.

I know I can do this by setting it up when I write the post, but I don't want to go back and have to edit old posts. I looked through a couple themes that do this, but can't seem to get it to work on the theme I am using.

I'm sure I am missing some little thing, usually the case. I've also spent quite some time searching the WP site but can't seem to find the answer.

Any suggestions?
