I need html/php Work Done With User Data Base
 

I need some html/php work done (form) type of thing that sends form info the user submits to a data base then outputs a progress bar graphic telling user they have completed the actions required. I will also need a back end to login and view the info submitted.

Qualifications:
PHP and MySQL
Captcha (added)
3 Day Time line

Please ICQ me with a price quote of what you can do this for. I have no time to do this so I wish to outsource this small project. You don't have to live in the US to do this project but I would prefer US programmers only.

Ok I figured this out and was able to do it, but now I need someone to design my CSS. So if your good at CSS please ICQ me asap. My CSS needs to enhance my input field boxes and I also need Ajax work done for a progress bar that I need to show on my page after a user submits data.
