Broken form on bestxxxfucksite

Some of you may know owner of this LL... When I'm trying to submit, the form can not take my uploaded image (The uploaded preview thumbnail is not a valid upload file). Anyone else have this error? Who owns this LL?

repaired the table, but, I don't see any event that would have caused it.
