It seems either exit to non adult site or my text ad at main page is counted as multiple outgoing links.

Any help would be appreciated

I'm not the one that reviewed your site so I'm just guessing but I count four links on your warning page. Actually since one of them is an exit link it is kind of borderline.

The ad on the main page looks like a bunch of ads even if it isn't so it may have gotten flagged by mistake.

Again I would say go ahead and resubmit and I'll get it in for you.
