#4 Seems like a game. Give the submitter one accept and the submitter will give you a thirty or forty free recips until he/she gets wise.
#5 Just no news of any kind from these people, do I send them an email and beg for rejection just so I know where I stand?

If you're talking about freesites I'd give it a good 2 months in the case of #5 if it's a LL you really want to get listed on. They may be working through a long backlog and sometimes the wait really pays off.
As for the other cases you mention it has to be your judgement call. I tweak my submit list all the time based on acceptance/rejection/my stats. Ideally I would like to submit to all the LLs out there. Unfortunately there's not enough hours in the day and my style of building freesites isn't always going to match up with the preferences of the reviewers.
