The DMCA violation can be sent directly to Google and Yahoo. I have done so on at least two occasions and the infringing sites were removed from the index within a matter of weeks. Here’s the Google link: http://www.google.com/dmca.html#notification

Once Google finds in your favor this is what the search will look like (note the bottom of the page):

The same can be done for the index proper. I noticed someone filed a DMCA violation that applied to one of my targeted SERPs. Check out (again notice the bottom of the page):

I did not get an approval notice. The DMCA notification just showed up in the SERP. However, on another matter, when Google disagreed with me they did e-mail to suggest that I provide more information.
