thats just a simple form...nothing fancy there..once completed it just sends the results to you in an email.

If you don't know html, most WYSIWYG html programs have a forms function..if you dont have that... try google I found several free ones using the search term "form creator"

what exactly do you want to do with the form is the question. Insert the values sent into a database would be my first guess?
