JW Player's page has an example along with the code download.

That being said I still use .mpg files on free sites since the galleries are recycled from making TGP galleries and some of the TGPs that I submit to don't allow embedded videos but it is what I use on all the paysites that I manage. That and no player is needed for mpg although they don't play on mobile devices like the iPhone.
