Error submitting to LOR

When I submitted to LOR today I got the following error (I filled in all fields correctly!)

"Your Name is required.
Your Email Address is required.
The Site Name is required.
The Site URL is required.
The Site Category is required.
Your Comments are required.
A Reciprocal Link is required.
The Site Sub Category is required."

I've had this problem once before and for the next 6 months I was unable to submit because I was getting this error msg

GG, do you know what's causing this?
