If you have multiple index templates, just mix them up more. I struggle with the index too, you are not alone. I would suggest not linking the all of the lines of those text ads. That's one of things many link lists are getting tired of. Just link one line or the name of the site the surfer is going to.

Since we're on the subject of index pages, I'd like to point out that the index is probably the page that I scrutinize the most because I think it's the page which is most confusing for surfers, and it's also the page we are more likely to make sales from. Many of us are guilty of just sort of tossing the index together and spend most of our time on the main and gallery pages. We have to balance the fact that it could be our highest click-thru page, but also providing clear navigation for surfers who may never buy porn. (bastards)
