Both. I'm currently paying for worldsex,richards-realm,shemp,and easypic. I tried buying some through SubmitPasses.com but they're a pain in the ass. All others are free accounts, though some of the best ones nowadays are closed to new submitters or you'd need to buy a pass. 90% of the traffic comes from 10-20 tgps I think.
