Minimum payout: $25
Commission on my book: 35% however note:

2) If you advertise books other than mine the author chooses the percentage so it may be different. Most pay less, a few pay more.
I'm not sure what "etc." you want, all I can think of is:
Promotional tools (on my book, others do not supply as much):
Various size banners
Text ads
Full page ads
Combined text and graphic ads
SmashWords widget (hosted script thingy)
Some (not a lot) photos
Coming next week: re publishable articles, story extracts, interview, etc - ideal for blog traffic and content.
