Submit form does not work no clue why

I just got notice from a submitter that my submit form does not work. I checked into it filled it out and press submit and I get nothing. No errors or anything. I'm using autogallery pro.

Have you made any changes at all to that pages content or structure lately?

You have your recip code <textarea> withing a <form> for no obvious reason. Remove the <form method="POST"> from that and the </form> and try. Perhaps that is 'confusing' the actual submit form. Maybe?
