IMO it should be on every page, though I see some people doing it only on the warning page.
From a SE perspective, every page is a potential landing page, and rarely do freesite builders put links pointing back to the warning page. I don't know if it'll cover you, but if you don't want to put an actual "a href" you can list the url as straight, non-clickable text where the link would be, and brief instructions saying to go to that page to get the 2257 info.
