ErrorDocument 401 cannot be a URL -- it must be a local path, i.e.
ErrorDocument 401 /401.html otherwise, it writes all sorts of strange errors in the log. Additionally, if you are missing that leading /, older versions of apache will write the same message, but, is very misleading.
