Getting your own Merchant Account will likely be difficult for extreme fetish.
Mitch@Netbilling would be one person to check with -- I know Visa/MC recently put some regs in that would make it more difficult for extreme fetish. Other than that, you'd probably need to go offshore which would be quite costly.
