I had debated whether to make it totally anonymous which I felt would get more honest answers, or, asking for some way to identify people to see whether their answers fit my assumptions. I went with anonymous because I figured some people might hold back from giving me their opinions which would invalidate some of the results if people told me what they thought I wanted to hear.

I told him this question should have included previous hosts as well and signed it "Have you ever lost data due to a hardware or software issue at your current host?"
