Can't fix the latter, but as to the first you should consider bifocals.
I have two prescriptions, one is for progressive bifocals for distance and reading which is my normal every day prescription...but I also have computer bifocals for the 24-30 inch distance computer monitors and the 12-16 inch distance keyboard as half my day is watching trading charts on computer screens
